CHAMPSS (Choosing Healthy Appetizing Meal Plan Solutions for Seniors) allows those age 60 and older to choose from different entrees and eat at their convenience seven days a week at CHAMPSS locations throughout Shawnee, Jefferson, and Douglas counties. This program is for active persons who are not home bound and can go to the CHAMPSS site to eat their meals. Participants must complete an orientation and submit a registration packet to enroll in the program. Participants are also encouraged to make the suggested donation of $3.50 per meal. No one is denied a meal if they are unable to donate.

To become a CHAMPSS participant you must complete all steps below to join the program:
1. Use your mobile device or computer to watch the CHAMPSS orientation video:
This 18-minute video provides information about JAAA services and the CHAMPSS meal program.
2. Complete the enclosed Uniform Program Registration (UPR). This form is required to participate in the program which is partially funded by Federal money and partially by donations of those who participate in the program. This form requires the signature of the participant. This form can be downloaded below or sent by mail. To get this form by mail please call: 785-235-1367
3. Complete the CHAMPSS Meal Order Form. Only 12 meals can be added to your card at a time. There is a suggested donation of $3.50 per meal. Some give more or less according to their ability. All donations are appreciated. Meals expire 12 months from the date they are added to the card. Donations are NOT Refundable only add meals you can use.
4. Return the completed forms with your donation. These may be returned in person or by mail. Please call us if you prefer to donate with a different method.

Renewal Information - Downloadable
This renewal form registers you for another year of the CHAMPSS program and you will be able to continue using your CHAMPSS card. This form is required by the state of Kansas to be completed each year. Meal credits can only be used if your account is active.

Reminder: Meal credits expire one year after being added to a CHAMPSS card.

If you are donating with a Vision card, you must come to the Jayhawk Area Agency on Aging (JAAA) office at 2910 SW Topeka Blvd, Topeka, KS 66611.
